The pediatric emergency room at Santa Casa State Hospital, in Cuiabá, already operates at 130% of its installed capacity. The situation was presented to the Regional Council of Medicine of Mato Grosso (CRM-MT), which held a meeting this Wednesday afternoon (24.04) to discuss pediatric care in the Baixada Cuiabana region.

According to the director of the State Hospital, Patrícia Neves, the unit’s emergency care went from a thousand to 3 thousand visits in one month. The manager explains that it is necessary to reorganize the pediatric care flow in Cuiabá, so that the unit does not become overloaded.

“Our services take place 24 hours a day, with the door open, and it is always busy. We have to reinforce the flow that is established by the Unified Health System (SUS), since the Santa Casa State Hospital, a unit referred to for medium and high complexity, cannot also take on primary care activities. If everyone commits to doing their own thing, everything is fine”, he assessed.

Distressed, she argued that the State Hospital, which operates specifically in medium and high complexity, is mostly meeting demands that should be absorbed by municipal units.

“We arrived at the triage and found that the majority of the fixed ones are blue, which means that they are demands that could have been resolved in the municipality’s primary or secondary care. We are running a serious risk of compromising the care of a critically ill patient because of this system dysfunction,” he assessed.

During the same meeting, the pediatrician at Santa Casa State Hospital, Dr. Vanimar Júnior, reinforced the director’s request and exposed the reality experienced by the unit’s professionals.

“We are already collapsed. Between 80% and 90% of the records we service are classified as blue, they are basic unit records. The mothers who go there [na unidade básica] They don’t find service. We need to have an aligned flow”, declared the professional when referring to the service flows established by the SUS.

Population service flows

The Deputy Secretary of Hospital Management at SES, Oberdan Lira, was present at the meeting and reinforced that the public health network’s service flows are pre-defined by the SUS.

“We are going through an epidemic period, full of respiratory syndromes and arboviruses, and this impacts the demand for health units. At this moment, we have to think about joining forces and organizing the flows of care for the population, carrying out an effective screening of patients, so that they are properly cared for in units prepared for that type of care”, he highlighted.

The manager considered that patients with mild flu-like symptoms, such as cough and runny nose, should be treated at the Health Center or Basic Unit. The same flow occurs if the mother, father or guardian of the child needs, for example, a medical certificate for the patient.

Children with a high fever, above 38 degrees, or more serious symptoms, such as acute pain, should be taken to Emergency Care Units (UPAs) or Polyclinics.

The emergency services of highly complex hospitals, such as the Santa Casa State Hospital, are prepared to care for children with complex health conditions, such as cancer patients who have flu-like symptoms or patients with acute pain that suggests infection.

“The population needs to pay attention to these guidelines. We are overcrowding a tertiary unit and could harm the care of aggravated cases at the expense of less serious cases,” she concluded.
