With the aim of providing a personalized service to the public that needs special, high-technology and high-cost products, acquired by the National Immunization Plan (PNI) of the Ministry of Health (MS), the State Department of Health (SES- TO) has had the Reference Center for Special Immunobiologicals (CRIE) since 1999. The service benefits a special portion of the Brazilian population who, for biological reasons, are prevented from using immunobiologicals available on the public network or require special products.

In Tocantins, there are currently two CRIE’s, in Palmas and Araguaína, which serve the State’s 139 municipalities. All face-to-face service is carried out in the macro-region of Araguaína, with service to the general public focused on the demand for special vaccines, consultations and the application of special vaccines. The Palmas macro-region serves virtually, the patient’s municipality of residence sends the request for a special vaccine with a report and all the information necessary to release the immunobiological. If approved, the adult and pediatric vaccine is administered at the Hospital Geral de Palmas (HGP) or in the municipality of residence of the requesting patient.

According to the Immunization Manager at SES-TO, Diandra Sena, “CRIE is a SUS service dedicated to the free immunization of people who are at greater risk of becoming ill and developing serious forms of infectious diseases. It offers the entire structure and health professionals specially prepared to care for patients living with chronic diseases, immunocompromised due to illness or treatment, as well as those who live or have close contact with these groups”.

Diandra adds that “through CRIE, the Secretariat is able to have a fixed and established service, aimed at patients and populations established and defined by the Ministry of Health, who cannot receive the other vaccines in the National Vaccination Calendar. So, it is a population that needs to have this special care with special immunizations through an environment with humanized, welcoming care. They are adults, the elderly and, mainly, children who for some reason cannot receive these vaccines from the normal routine vaccination schedule”.

“We are committed to expanding protection for this public because the degree of illness compromises their quality of life and receiving these vaccines will guarantee a safe and hospital-free day to day life. The moment is to bring the vision of individual reception according to the diagnosis”, reported the CRIE/SES-TO nurse, Telma Regina Rodrigues.

The Create

Created within the scope of SES-TO through Ordinance – 21/2020/SES/GASEC, established based on general guidelines for operation, the Reference Centers for Special Immunobiologicals (Crie). Its competence is to care for people with congenital immunodeficiency, solid organ and bone marrow transplants, HIV carriers, neurological diseases, heart disease, pneumopathies, hematological diseases, among others. Furthermore, it supports the investigation of events supposedly attributable to vaccination and immunization (Esavi).

To use immunobiologicals, patients must submit a medical request with a clinical report on their case, prescriptions and patient identification documents for case evaluation. These indications will be evaluated by the doctor or nurse responsible for Crie and the immunobiologicals dispensed.
